Use this before every clinic, teleconsult, or follow-up appointment to share a clear, one-page summary of medicines, trends, symptoms, and decisions needed from the doctor.
When to use this template
- 24 hours before a scheduled doctor visit or teleconsult.
- Any time there is a medicine change, new symptom, or sudden decline.
- At discharge follow-up visits where the doctor needs a concise home-care summary.
- Before specialist referrals (cardio, neuro, diabetes, rehab) to avoid repeating history.
How to use it (5-step workflow)
- Collect last 7 to 14 days of logs (daily readings, BP/sugar, medicine adherence).
- Fill patient and visit details first, including reason for visit.
- Write current medicines and clearly mark what changed since last review.
- Summarize trend highlights and red flags, not every single reading.
- Prepare 3 to 5 specific questions so the consultation ends with clear next actions.
How to fill each section quickly
- Patient & visit date: Name, age, diagnosis, visit date/time, doctor name, and primary concern.
- Medicines (current / changes): List active meds with dose and timing; mark started/stopped/changed items.
- Vitals & symptoms since last visit: Include trend statements (for example, morning sugar improving, evening BP elevated) plus symptom frequency.
- Questions for the doctor: Ask decision-focused questions on dose changes, warning signs, tests, and next review date.
