How to turn the answers into the care routine
If the doctor changes medicines, monitoring frequency, or warning thresholds, update the Care Routine Setup template after the visit so caregivers are all following the same plan.
Practical family rule
Do not carry every raw reading to the doctor. Carry the trend, the concern, and the decision needed. That makes the visit faster and the next steps easier to follow.
